Photo/Video Release Consent

I hereby grant the organizers of the Arts and Culture Festival and its authorized representatives (“Organizers”) the irrevocable right and permission to photograph, film, and/or record my performance and likeness during the festival. I understand and agree that:

• These images, videos, and recordings may be used for promotional, marketing, educational, and archival purposes
• Usage may include but is not limited to: social media, websites, print materials, press releases, future event promotion, and grant applications
• No compensation will be provided for such use

Liability Waiver Agreement

By participating in the Arts and Culture Festival (Festival), I acknowledge and agree that:

• I understand that participation in this festival involves inherent risks, including but not limited to physical injury, property damage, or other losses
• I voluntarily assume all risks associated with my participation
• I release and hold harmless the Festival, its organizers, volunteers, venue owners, sponsors, and affiliated parties from any and all liability, claims, demands, or causes of action arising from my participation
• I am responsible for my own safety and the safety of my equipment, props, and personal belongings
• I have adequate insurance coverage for any potential injuries or damages
• This waiver is binding upon my heirs, executors, and assigns

Performance Terms & Conditions Acknowledgment

I acknowledge and agree to the following performance terms:

• I will arrive at my designated performance time and location as scheduled
• The Festival is family friendly, aimed at promoting traditional arts and culture, and that my performance is appropriate for all audiences consistent with the Festival theme
• I am responsible for providing my own equipment, instruments, and technical requirements unless otherwise arranged
• I will respect other performers, festival staff, volunteers, and attendees
• I understand that inappropriate behavior may result in removal from the festival
• Performance schedules may be subject to change due to weather or other circumstances
• My performance does not infringe on any copyrights or intellectual property rights
• I have the legal right to perform all material in my act

Minor Participants Agreement

If my performance includes any participants under the age of 18,

• I acknowledge and agree that:
  I assume full responsibility and liability for all minor participants in my performance
• I have obtained proper written consent from the parent(s) or legal guardian(s) of each minor participant
• I have informed the parent(s)/guardian(s) of all festival terms, conditions, and potential risks
• I will supervise all minor participants throughout their involvement in the festival
• I release and hold harmless the Festival from any claims, damages, or liability arising from the participation of minors in my performance
• I am responsible for ensuring that all minors comply with festival rules and guidelines
• I understand that I may be held legally and financially responsible for any actions or injuries involving the minor participants
